Calcineurin inhibitor

Start at 0.5 mg BID or 1 mg. BID. Increase based on trough levels. Typically max dose would be 10 mg. BID. Typical dosing is 1-3 mg. BID. Trough level should be in AM before AM dose. Dose 12 hours apart NOT BID so 7 AM and 7 PM, 8 AM and 8 PM. Steady state is attained after about 6 half lifes so eery 3-4 days after a dosage change.
